MEDICAL  HOME  COMMITTEE

Medical Home Committee Brochure
Joint Principles of a Medical Home
Family-Centered Medical Home

GOALS

  • Every child in Arizona should have a medical home with care that is accessible, continuous, comprehensive, family-centered, coordinated, compassionate and culturally effective
  • Access to healthcare for all Arizona children, including uninsured, underinsured, and insured children will be enhanced and guaranteed

ACTIONS of the MEDICAL HOME COMMITTEE 2009

  • The creation of an AzAAP website that can be used by all AZ physicians who provide care for children.  This website will be a website that will help pediatric care providers to provide a medical home.  The website will have phone numbers, addresses and contact information for appropriate resources that pediatricians can use, in particular to help navigate the complex system of development and behavioral referrals
  • The Medical Home Committee has partnered with the Pediatric Council to aid in payment reform for pediatric practices to fully implement the medical home
  • The Medical Home Committee is involved with seeking solutions to improve access to health care for children who are privately insured, uninsured, eligible or ineligible for AHCCCS
